Cars and vans tend to have holes left in the body after the production process and these are usually filled in with rubber grommits.
Press the patch into the hole firmly.
Now you may begin welding it 2 inches at a time.
It s pretty easy when you know how.

Tack weld the patch in place.
Use your fingers to flatten and smooth the patch against the area of the car so it follows the contour lines of the vehicle.

Hold the patch in place with one hand and use your mig welder to tack weld all four corners of the patch on the outside of the vehicle.
Weld both inside and out.

Alternate sides while welding to prevent the floor from warping.

Having removed the bolts and rivets i was left with all these holes to patch.
It could be that your van is missing one of these or as mark mentioned in the comment on your question it could be missing something like a screw or something used to be screwed in there.
